Subject Access Requests

If you want a copy of your medical records, please contact us online. If you only need part of your records, let us know which parts and time periods you need. You have the right to see your medical information, but you cannot get information about other people, like family or friends. This information will be removed.

Requests are usually done within 28 days, but sometimes it might take longer. We will update you on your request’s progress. The time it takes depends on how complicated the request is, when it was submitted, and staff availability.
