New Patient Registration
Register with the practice as a new patient
How to Register
Check you live within the boundary area
Our practice serves the areas of St Albans, Redbourn, Flamstead, Markyate, and part of Woodhall Farm in Hemel Hempstead. We do not register new patients from Harpenden, Hatfield, Chiswell Green, or some parts of London Colney, as other practices already cover these areas well.
Out of area
If patients are moving out of area they can still stay registered with the practice if they wish but they will not qualify for home visits.
To access our registration process, first check you are in our catchment area. Keep in mind that sometimes new areas might not show up on the postcode tool right away, like in new housing developments. If you’re not sure, check the map or contact us for more details.
Temporary Resident Registration
You can register as a temporary resident if you will be living near the GP surgery for up to 3 months. After 3 months, you will need to register as a permanent resident if you plan to stay longer. You can register temporarily if you are away from home for work, study, or vacation. You will still be registered with your main GP surgery, and we will send any details of your treatment to them so they can update your medical records.
Temporary Registration for University Leave
If you are back from university and need to register temporarily, make sure to fill out all the fields correctly:
- Your home address should be your university address.
- Your GP surgery details should be the GP you see at university.
- Your temporary address is where you are staying while you are on leave from university.
This helps make sure you are registered correctly and don’t get removed from your main GP at university. If you don’t complete all the fields, it might delay your registration.

Register a Carer
It's important for us to know if you are a carer so we can give you the right information and support.
If you are a carer, please fill out the following form.
Change of Personal Details
Please tell us right away if you change your address, phone number, or name. It’s important for us to have the correct details so we can contact you if needed.
If you’ve changed your name because of marriage or a legal name change, please give us a copy of the document that shows this (as required by the Department of Health).
Your Named GP
Every patient at our practice has a specific GP who is responsible for their care. When new patients join us, they will be told who their named GP is. If you don't know who your named GP is, just ask a member of our reception team.
For non-urgent problems, it's a good idea to see your named GP for better care. However, if you can’t see them, you can book an appointment with any of our other doctors or clinicians.
