Medical Report Request/Private Letter
If you would like to request a medical report, please visit eConsult to make your request. It is important to specify what information you require or how many years of records are needed. There may be a charge for this service and can take up to 28 days for you to receive your report.
The practice has a primary duty to provide services for the management of patients who are ill. Private letters and medical reports fall outside of the NHS GP contract and as such, have to be completed in the GP’s own time outside of their paid NHS working hours. These services may incur a fee which we set with reference to the British Medical Association guidance on private fees.
If you require a private letter or report from your GP we require payment for this service, please find our fees for private work below:
Payments: Please be aware that the surgery no longer accepts cash where payments are required. All payments will now only be processed by card. Once payment has been made by card the request will be sent to the GP to complete, this service can take up to 28 days.
Subject Access Requests
If you require a copy of your medical records, please complete an econsult form. If you do not need your entire record, it would be helpful if you would inform us of the periods and parts of your health records that you require, along with details which you may feel have relevance. You are entitled to have your medical information. You are not, however, entitled to receive any third party patient identifiable information. This includes any information pertaining to: spouse, partners, siblings, family members, friends etc. This information will be redacted.
Requests are usually completed within 28 days but under certain circumstances, this time period can be extended but we will keep you informed of the progress of your request during this extended period. Timescale is dependent upon the complexity of the request, time of submission, and the availability of staff to complete the request.
