Patients must put their request in writing, either by email to firstname.lastname@example.org or by letter. In the request, patients must confirm which part of the record is required:
- Electronic only
- Electronic and paper notes
- Specific part, in which case precise details eg date range, condition, need to be stated
The practice requires 28 working days to supply the records. Once complete, the practice will either send the records in the post or advise patients to collect them from the surgery.
